Abstract Speech is of particular importance for patients with cleft lip and palate, which can cause the functional speech deficit known as velopharyngeal insufficiency. Despite significant progress in the management of cleft lip and palate and associated deformities, the speech results are not yet completely satisfactory. This paper reviews the history of the management of cleft lip and palate and describes a new surgical procedure for repair regarding a new physiologically based approach.
